REQUEST A CHANGE TO YOUR STUDENT'S LEARNING MODEL
  • Please complete this form for each student if you would like to request:- a change from Group A to Group B OR- a change from Group B to Group A OR- a change from Hybrid to Remote OR- a change from Remote to HybridThe deadline to request a change is January 29.Changes are effective March 1. Your school will contact you before March 1 to confirm that a change request has been granted, or to notify you in the rare event that a change request cannot be granted.
